Multiple sclerosis (MS) is currently the most prevalent disabling neurologic
disease of young adults in America (DeBolt & McCubbin, 2004). It causes
demyelination of nerve axons, which can lead to lack of normal action potentials as the
central nervous system (CNS) attempts to communicate with the body, and vice versa.
Common symptoms of MS include decreased walking ability and balance, as well as
increased muscle weakness and fatigue. These symptoms and several others are not only
detrimental to general health, but the ability to perform routine life activities, such as
those mentioned above, and many other everyday motor movements (White et al. 2004).
Exercise is a low-cost form of therapy that has been gaining recognition as a
realistic and positive way for people with MS to care for themselves. Intervention with
an exercise program has the potential to improve many of the impairments that plague
victims of this disease, yet limited research has been done to determine the extent of
progress that can be made through physical training. Aside from being pure physical
therapy, exercise tends to positively affect mood and confidence in most people, as well
as the extent to which sufferers of multiple sclerosis actually feel disabled—a mental
state that can be particularly crippling in its own right.
Articles reviewed in this paper investigate the effects of aerobic exercise, weight
training, and resistance training on walking ability, muscle strength and fatigue,
functional ability, and quality of life for people with MS. Each experiment was
structured differently with regard to structure and content of exercise program, as well as
exact measures and outcomes, however all of them conclude with the findings that
exercise can result in some form of improvement.

Aerobic Exercise Improves Walking Ability, Feelings of Fatigue & Disability
Kileff and Ashburn (2005) studied the effects of aerobic exercise alone on
walking ability, balance, fatigue, and self-reported disability in MS patients. The
measures used to rate these factors were the 10-meter and 6-minute walk tests, the
Functional Reach (an indicator of balance), and the Guy’s Neurological Disability Scale
(GNDS) for report of disability. Eight female subjects were recruited to the study, but
one had an injury and another relapsed, so data from only six were used.
The exercise consisted of were 30-minute, bi-weekly sessions of static bicycle
pedaling at each individual’s highest level of exertion. Measures were taken before the
program and then again after 24 sessions were completed, and significant improvements
were made in the six-minute walk, GNDS, and self-report of fatigue. None of the other
tests were significant, yet all showed possible trends of improvement rather than
decrement.
The researchers admit that this was more of a preliminary study to use as a basis
for further clinical experiments, and that both the extremely small sample size and lack of
a control group limited what could be concluded from the results. However, the
improvements in mobility and feelings of disability were excellent for a simple program
of an hour-per-week commitment, especially since all subjects reported feeling better
physically and mentally at the end of it.
Resistance Training Programs Improve Walking and Leg Strength
Since many studies on exercise and MS had been constructed similarly to the one
above, basing their programs around aerobic exercise, White et al. (2004) looked at the

effects of the resistance training program. The aim of the project was to measure changes
in leg strength, walking ability, and self-reported fatigue and disability.
The tasks used to measure strength were voluntary isometric weight tests for knee
extension, knee flexion, and plantar flexion. The tests for functionality included a 25-
foot timed walk, 3-minute step test (how many steps a participant can go in 3 minutes), as
well as questionnaires for self-report of fatigue and disability. Training volume was also
measured, as the sum of maximal training weight for 10 repetitions of knee extension and
flexion completed for all participants each week.
Eight subjects were recruited, had baseline measures taken, and were all put on
the 8-week, bi-weekly resistance training program. After completion of the program
(with 100% adherence to workouts), all measures were taken again.
Results from this experiment showed improvement in strength for knee extension
and plantar flexion exercises, as well as better stepping rate in the 3-minute step test.
Self-reported fatigue decreased significantly, along with self-reported disability. The
results of increased walking ability and decreased fatigue and disability all concur with
results from aerobic exercise experiments, which suggests that type of exercise may not
be as important as the mere act of exercise for physical and mental gain to take place.
The increase in leg strength is yet another positive effect seen with the resistance training
program, and it would be interesting to see if the leg strength improves in aerobic
exercisers as well.
The Effects of a Long-Term Exercise Program on MS
While the previous two experiments were relatively short-term and included a
minimal number of subjects, Romberg, Virtanen, and Ruutianen (2004) investigated the

improvements that occurred in a larger number of MS patients (95) after intervention
with a more intensive program lasting at least twice as long. The authors studied the
effects of the intervention on both functional impairment and health-related quality of life
(HRQOL), mainly inspired by a lack of information or prior research of how exercise
changes HRQOL in MS. The tasks to test functional impairment were a timed 25-foot
walk test, nine-hole peg test, and paced auditory serial addition test. HRQOL was tested
using the Multiple Sclerosis Quality of Life-54 (MSQOL-54) questionnaire.
Subjects with mild to moderate disabilities due to MS were recruited, stratified by
sex, and randomly assigned to either the exercise or control group. Baseline tests were
performed, and then the 26-week training program was initiated in which resistance
training with elastic bands occurred 3-4 times per week, and aerobic training took place
once per week. Subjects were taught the exercises for the first three weeks, and then
continued the program at home for the following 23 weeks. Control subjects received no
intervention and were asked not to change their physical habits during the experiment.
Results showed statistically significant improvement for exercisers in the 25-foot
timed walk, as well as significant decrement in the control group for the 25-foot time
walk and nine hole peg test. This showed that exercisers were getting better or at least
stayed the same while non-exercisers were getting worse. There was no significant
difference in HRQOL scores. These same researchers reported, in another paper on the
same experiment (2004), that the exercisers also significantly improved in upper
extremity endurance and walking speed in the 500-meter walk test.
Aside from confirming that this exercise program does increase walking ability in
people with MS, which agrees with many other papers on this topic, these results also

confirm that exercise is not harmful or detrimental to such individuals. The non-
significant results regarding health-related quality of life test scores was mentioned
because of the possible alternative explanations for this result. The MSQOL-54
questionnaire may not have been an accurate measure of quality of life, as the definition
of such is relative to the questions being asked. There are many other questionnaires or
measures of quality of life that may have yielded different results, although a self-report
would be good because it comes from the perspective of the individual being studied.
The Effects of Resistance Training on Leg Power and Balance
By measuring walking ability, all of the studies discussed this far have also been
measuring balance in an indirect and elementary way, as an individual who has poor
balance due to multiple sclerosis won’t be able to walk very well. In this final article,
DeBolt and McCubbin (2004) test the effects of a resistance workout program on not
only mobility and leg power, but on balance directly as well. Measurements used were
the Up and Go test for mobility (stand up from chair, walk 3m, walk back and sit down),
the Leg Extensor Power Rig for leg power, and the AccuSway platform for balance
(measured direction and velocity of sway while standing).
There was a 2-week instruction period followed by 8-weeks of at-home training, 3
times per week. The exercises were all simple weighted leg activities, such as standing
up from a chair, lunges (wearing a weighted vest), heel-toe raises, and leg curls. All
subjects adhered to the program well and measures were taken before instruction and
after completion of the program.
Results showed that only leg extensor power increased in the exercise group,
while neither balance nor mobility changed for either group. Again, no injury or increase

in MS symptoms occurred during the program. While the results of this study are not as
impressive as some other research has been, the results that were significant are still
positive. It was a good idea to attempt to measure balance directly, and a change of
training program may show a different result in future studies.
Conclusion
All of the experimenters each found that sufferers of MS experience some
positive impact after completion of an exercise program. Mobility seemed to be the
primary focus of most of these researchers, and was the most typical factor of
improvement in subjects. As mentioned earlier, improvement in mobility may also be
interpreted as an indirect improvement in balance, as people generally walk faster when
they have better balance standing up. While exercise is an excellent form of therapy for
physical reasons, the mental and emotional implications that may accompany a good
workout are important as well. As seen in these studies, those who participated in the
exercise program usually felt less fatigued and disabled afterwards, which can build
confidence and a better attitude towards the unfortunate situation that many people with
multiple sclerosis face.
Furthermore, data on MS samples have indicated that most people with MS die
from similar ailments as people in the general population, such as coronary heart disease
and stroke (Kileff & Ashburn, 2005). This gives rise to the notion that exercise,
especially cardiovascular exercise, may improve the lifespan of the individuals with MS
as it seems to with the general public.

   بیماری های MS اخیرا شایع ترین ناتوانی عصبی در بین جوانان آمریکایی است (دبلت مک کابین 2004) باعث از بین رفتن عصب می شود. که می تواند منجر به فقدان پتانسیل های عمومی مثل سیستم عصبی مرکزی (CNS) شود که برای ارتباط با بدن مورد بررسی قرار میگیرند و برعکس. علائم عمومی MS عبارتند از کاهش توانایی راه رفتن و تعادل ، افزایش ضعف و خستگی ماهیچه .

   این علائم و چندین نوع دیگر نه تنها برای سلامتی مضر هستند بلکه فرد قادر به انجام فعالیت های روزمره که در بالا ذکر شد و بسیاری از حرکات روزمره دیگر نسیت.

   ورزش شکل کم هزینه درمانی می باشد که راه مثبت و واقعی برای افراد   MS  تشخیص داده شده است. دخالت در برنامه ورزشی پتانسیل را برای پیشرفت خیلی از قربانیان این بیماری به همراه دارد. تحقیق و بررسی می تواند مقدار پیشرفت بدست آمده از طریق آموزش فیزیکی را تعیین می کند. گذشته از این درمان فیزیکی ، ورزش اثر مثبتی رو رفتار و اعتماد در بیشتر افراد می گذارد به همان اندازه که از بیماری رنج می برند  حالت ذهنی می تواند مخصوصا منجر به فلج گردد مقالات تجدید نظر شده این صفحه اثرات ورزش ایروبیک را بررسی می کند. آموزش مقاومت روی توانایی راه رفتن ، خستگی و کشش ماهیچه ، توانایی کاری  و کیفیت زندگی برای افراد MS .

   هر آزمایش به طور مختلف ساخته شده است با توجه به ساختار و متن برنامه به خوبی اندازه گیری و نتایج دقیق بدست می آید به هر حال همه این نتایج حاکی از آنست که ورزش می تواند در بعضی پیشرفتها نتیجه دهد.

   ورزش آیروبیک ، توانایی راه رفتن ، احساس خستگی  و ناتوانی را افزایش می دهد.

کیلف و آشبرن (2005) اثرات ورزش آیروبیک را به تنهایی روی توانایی راه رفتن ، تعادل ، خستگی و ناتوانی که توسط بیماران گذارش شده بود را مورد مطالعه قرارداده اند و اندازه گیری ازمایش پیاده روی برای نرخ این فاکتورها 10 متر و 6 دقیقه ( تحقیق کاری ( یک فشار سنج تعادل) و نمره ناتوانی عصبی پسران برای گزلرش ناتوانی ) بود . 8 بیمار مونث برای مطالعه به کار گرفته شدند اما یک نفر آسیب دیده بود و دیگران را برگردادند و فقط اطلاعات را از 6 نفر به دست آوردند. تمرینات در جلسات 30 دقیقه ای به مدت 2 هفته روی رکاب زدن با دوچرخه ثابت و بالاترین فعالیت فردی انجام شده بود .اندازه گیری ها قبل از برنامه گرفته شده بود و سپس دوباره بعد از 24 جلسه کامل شده بود و پیشرفتهای مهم در پیاده روی 6 دقیقه ای ایجاد شده بود و میزان خستگی توسط بیماران گزارش شده بود .

هیچ یک از آزمایشهای دیگر مهم نبودند هنوز هم پیشرفتها نسبت به میزان کاهش نشان داده شده ان محققان تصدیق کردند که این تحقیق بیشتر از مطالعه اولیه برای استفاده به عنوان اساس آزمایشات کلینیکی است. و هر دو اندازه نمونه بی نهیت کوچک و فاقد یک گروه کنترل محدود است که می توناند شامل نتایج شود به هر حال پیشرفتها در تحرک و احساس ناتوانی برای یک برنامه ساده 1 ساعت در هفته عالی بوده است مخصوصا زمانی که همه عامل ها (بیماران) گزارش دادند که در پایان دوره از لحاظ جسمی و ذهنی احساس خوبی داشتند. برنامه های آموزش مقاومت باعث پیشرفت کشش پا و پیاده روی می شود:

زمانی که خیلی مطالعات روی ورزش و به طور مشابه برای یکی از موارد بالا ایجاد شده بود ، بر اساس برنامه هایشان بر رو ورزش آیروبیک وایت ات آر (2004) اثرات برنامه آموزشی مقاومت را مورد بررسی قرار داد هدف پروژه اندازه گیری تغییرات در کشش پا ، توانایی راه رفتن و ناتوانی و خستگی بیماران بود.

کارهایی برای اندازه گیری کشش به طور داوطلبانه انجام شد آزمایشهای وزن برای کشش زانو ، خمیدگی زانو و خمیدگی کف پا .

آزمایشها شامل 25 فوت پیاده روی ، 3 دقیقه پله (هر شرکت کننده در عرض 3 دقیقه چند تا پله می تواند بالا برود) پرسش نامه هایی برای گزارش ناتولنی و خستگی افراد تهیه شده بود.

همچنین حداکثر وزن برای 10 بار تکرار کشش زانو و خمیدگی برای همه شرکت کنندگان در هفته در نظر گرفته شده بود.

8 شرکت مننده تازه کار آمده بودند تا مورد اندازه گیری قرار گیرند و برنامه آموزشی مقاومت 2 بار در هفته  وبه مدت 8 هفته بود. بعد از تکمیل برنامه همه اندازه گیری ها دوباره بدست آمد.

نتایج بدست آمده از این آزمایشات حاکی از پیشرفت در کشش برای کشش زانو و تمرینات ( ورزشهای خمیدگی کف پا ) بود.

به خوبی نرخ بالا رفتن از پله در 60 پله در مدت 3 دقیقه گزارش خستگی افراد بر طبق گزارش ناتوانی کاهش یافته بود.

نتایج افزایش توانایی راه رفتن و کاهش خستگی و ناتوانی با نتایج آزمایشات ورزش آیروبیک موافقت شد که پیشنهادات نشان  میداد که نوع ورزش احتمالا مهم نبود ، عمل ورزش برای ذهن وجسم مهم بوده است ، افزایش کشش پا هنوز با برنامه مقاومت اثر مثبتی دارد و جالب است که کشش پا در ورزش آیروبیک پیشرفت قابل ملاحضه ای داشته است.

اثرات برنامه ورزش طولانی مدت روی MS:

   در حالی که دو آزمایش قبلی در مدت زمان کوتاه و شامل تعداد اندکی از بیماران بودند .

روبرک ، ورت تاین و روتیاین در سال 2004 تحقیق کردند که پیشرفت های که منجر به تعداد بیشتری از بیماران MS شده (95) بعد از مداخله با برنامه شدید تر حداقل 2 برابر به طول انجامید.

نویسندگان اثرات مداخله روی هر دو ضرر کاری و کیفیت سلامت روی زندگی را مطالعه کردند به طور اساس استنشاق بوسیله یک  نقص اطلاعاتی یا تحقیق قبلی از چگونگی تغییرات ورزش در بیماران MS .

 کار آزمایش ضرر کاری 25 فوت تست پیاده روی و تست پا 9 و علاوه بر اینها شامل تست  شنوایی هم بود . این تست پرسشنامه هایی برای کیفیت زندگی بیماران MS نیز داشت ، بیماران با اداره کردن ناتوانایی ها برای بیماری MS  از هر جنسی (مرد وزن) از پیشرفت کار اضی بودند و به طور تصادفی  به ورزش و کنترل گروه می پردازند.

آزمایشهای baleline  (خط اصلی ) اجرا شد و سپس برنامه آموزشی 26 هفته ای به کار گرفته شد که آموزش مقاومت با بند های کش دار 34 بار در هفته اجرا می شد و آموزش آیروبیک هفته ای یکبار انجام می شد.

 بیماران ( عاملها ) ورزش ها را در 3 هفته اول فرا گرفتند و سپس برنامه را در منزل به مدت 23 هفته دنبال کردند  هیچ کس دخالتی در تمرین بیماران کنترل شده نداشت تا زمانی که عادت های جسمی در طول آزمایش تغییر کرد.

نتایج آماری بدست آمده از پیشرفت ورزش های 25 فوت پیاده روی  و میزان کاهش در کنترل گروه برای  25 فوت پیاده روی را نشان داد.

این نتایج نشان داد که ورزش ها نتایج خوبی داشتند یا حداقل بدتر از تمرینهای قبل نبودند . هیچ اختلاف مهمی در نمرات کیفیت سلامتی و کاری نبود.  این محققان در مقاله ای دیگر ازمایشی شبیه به این در سال 2004 اراوه دادند که تمرینات باعث پیشرفت تحمل بی نهایت بالا  وسرعت پیاده روی در تست 500 متر شد.  گذشته از این ، برنامه ورزشی در توانایی راه رفتن بیماران MS افزایشی نداشت که با دیگر مقالات در باره این موضوع موافقت کردند . این نتایج همچنین تاکید دارد که ورزش عنصر نیست یا برای این قبیل افراد عنصر است .

نتایج غیر ضروری به کیفیت سلامت ی زندگی توجه دارد زیرا توضیحات پیشنهادی ممکن برای این نتیجه بدست می آید.

پرسشنامه MSQOLS4 مربوط به تست سلامتی احتمال نداره ندارد که اندازه گیری دقیق کیفیت زندگی باشد.  به عنوان تعریف مربوط به سوالات پاسخ داده شده است.

پرسشنامه های زیاد دیگری هستند یا اندازه گیری های کیفیت زندگی که احتمالا حاصل نتایج مختلف است.

 اگر چه گزارش خود بیماران خوب بود زیا از مطالعات فردی استنتاج شده بود.

اثرات ورزش استقامت روی تعادل و قدرت پا :

   با اندازه گیری توانایی راه رفتن ، همه مطالعات برای اندازه گیری تعدل در یک روش مقدماتی و غیر مستقیم مورد بحث قرار می گیرد.

به عنوان فردی که از لحاظ تعادل ضعیف است و بیمار MS خیلی خوب قادر به راه رفتن نخواهد بود.

در این مقاله  پایانی تست دبلت  ومک کابین در سال 2004 ، اثرات برنامه استقامت روی تنها تحرک و قدرت پا نیست بلکه مستقیما رو تعادل است.

اندازه گیری ها برای تست بالا و پایین استفاده شده بود(از روی صندلی برخاستن ، 3 دقیقه پیاده روی ، پیاده روی و نشستن ) فدرت ماهیچه ای انبساط برای قدرت پا است و طرح اکسسوی برای تعادل (اندازه گیری مستقیم و سرعت در حین ایستادن) دوره آموزشی 2 هفته ای بود که تا 8 هفته آموزش در خانه دنبال شد 3 بار در هفته ورزش های مربوط به فعالیت های پا بودند از قبیل :

از روی صندلی برخاستن ، جهش ناگهانی ( پوشیدن جلیقه سنگین ) ، برخاستن روی پاشنه پا و پیچاندن پا .

همه عامل ها به خوبی موافق برنامه بودند و اندازه گیری ها قبل از آموزش و بعد از کامل شدن برنامه داده شده بودند.

نتایج نشان داد که تنها قدرت عضله منبسط در گروه ورزشی افزایش یافت در حالی که نه تعادل و نه تحرک این گروه را تغییر داده است .

دوباره هیچ صدمه یا افزایش در علائم MS در طول برنامه اتفاق نیفتاد .

در حالی که نتایج این مطالعه در سایر تحقیقات دیگر موثر نیستند.

نتایجی که مهم بودند هنوز هم مثبت هستند.

نظریه برای اندازه گیری تعادل به طور مستقیم خوب است و تغییر برنامه آموزشی ممکن است نتایج مختلفی را در مطالعات آینده نشان دهد.

نتیجه گیری:

همه آزمایش شوندگان فهمیدند که تجربه MS در بعضی ها مثبت بود و بعد از کامل شدن یک برنامه ورزشی بود .

به نظر می رسید که مرکز مقدماتی بیشتر این محققان تحرک باشد و بیشتر عامل معمول پیشرفت در عامل هاست همانطور که قبلا ذکر شد در تحرک ممکن است همچنین به عنوان پیشرفت غیر مستقیم تعادل تفسیر شود .

مانند افرادی که سریعتر پیاده روی می کنند و زمانی که تعادل بهتری برای برخاستن دارند .

در حالی که ورزش شکل عالی در مان برای دلایل فیزیکی ( جسمی) است. مفاهیم حسی و ذهنی که ممکن است یک کار مهم را همراهی کند .

با دیدن این مطالعات آنهایی که در گروه ورزشی شرت کردند معمولا خستگی و ناتوانی کمتری داشتنذد بعد از ان می توانند به خود اعتماد داشته باشند و تلاش بهتری به طرف موفقیت ناخوشایند دارند که خیلی افراد با بیماری MS روبرو هستند علاوه بر این اطلاعات بر روی علائم MS نشان داد که بیشتر مردم MS از بیماری های مزمن میمیرند مثل بیماری انسداد شرایین قلب  و ضربان قلب (گیلف و آشبرن 2005) این تصوریست که ورزش به ویژه ورزش کاردیووسکلار (مربوط به قلب) ممکن است مدت زندگی بیماران MS به نظر رسد که با عموم پیش رود.
